We know you have many choices when choosing a Periodontist in Ann Arbor, MI so we have made requesting an appointment a simple process via our Web site. If, for any reason you cannot keep a scheduled appointment, or will be delayed, please call us as soon as possible.
Insurance and Billing
We accept most traditional insurance plans, contact our office to verify acceptance of your plan. Mark D. Zahn, D.D.S., M.S., P.C. does not participate in Health Management Organizations; however, we will be happy to file your insurance claims for you.
We accept checks, cash or credit cards. We also offer a flexible payment plan. Please see our Financial Coordinator for details.
We are committed to providing periodontal care that is affordable for everyone. Your treatment plan will include a breakdown of all anticipated fees, and we will do our best to inform you of all costs before treatment is administered. Payment is required at the time services are rendered. For your convenience, we accept Visa, MasterCard and Discover and offer Care Credit as an outside financing option.
A member of our staff will review our payment options with you to find an option that best accommodates your financial needs. If special arrangements are needed, please speak with our insurance coordinator prior to receiving service.
Additionally, we do accept most insurance and we will be happy to file your insurance claim. We do ask that you kindly pay your co-payment at the time of service. We participate in Blue Cross and Blue Shield and Delta Dental Plan of Michigan, as well as Delta USA. We will fully attempt to help you receive full insurance benefits; however, you are personally responsible for your account, and we encourage you to contact us if your claim has not been paid within 30 days.
