First Visit

When you arrive for your first visit, please be prepared to complete all insurance and health information forms that will allow us to begin your dental treatment. We ask that you arrive 15 minutes before your appointment to allow time to fill out several forms that will get you acquainted with our office. Please bring any current X-rays you have or contact your referring dentist to have them sent to our office.

Your initial exam will last approximately one hour. Dr. Zahn will provide a review of your dental and medical history, as well as a thorough oral examination. If radiographs are needed, they may also be taken at this appointment. Dr. Zahn will then consult with you, outlining your specific oral problems and proposed treatment needs. If a second visit is indicated to provide you with the best treatment plan, a second consult appointment may be scheduled at no additional charge.
